BackgroundAs the population of the United States ages, chronic diseases increase and treatment options become technologically more complicated. As such, patients’ autonomy, or the right of patients to accept or refuse a medical treatment, may become a more pressing and complicated issue. This autonomy rests upon a patient’s capacity to make a decision. As more older, cognitively and functionally impaired individuals enter healthcare systems, quality assessments of decision-making capacity must be made. These assessments should be done in a time-efficient manner (...) at a patient’s bedside by the patient’s own physician. Thus, a clinically practical tool to assist in decision-making capacity assessments could help guide physicians in making more accurate judgments.ObjectivesTo create a clinically relevant Bedside Capacity Assessment Tool (BCAT) to help physicians make timely and accurate clinical assessments of a patient’s decision-making capacity for a specific decision.SettingThe Department of Medicine, Division of Geriatrics and Palliative Medicine, Zucker School of Medicine at Hofstra/northwell.ParticipantsGeriatric medicine fellows, palliative medicine fellows, and internal medicine residents (n = 30).MeasurementsSubjects used the BCAT to assess the decision-making capacity of patients described in 10 written, clinically complex capacity assessment vignettes. Subjects’ conclusions were compared to those of experts.ResultsThe subjects’ and experts’ assessments of capacity had a 76.1 percent rate of agreement, with a range of 50 percent to 100 percent. With removal of three complex outlier vignettes, the agreement rate reached 83.2 percent.ConclusionThe strong correlation between the two groups—one of physicians in training utilizing the BCAT and the other of specialists in this area—suggests that the BCAT may be a useful adjunct for clinicians who assess decision-making capacity in routine practice. The range indicates that further refinement and testing of this tool is necessary. The potential exists for this tool to improve capacity assessment skills for physicians in clinical practice. In this article, we discuss the current trend of authoritarianism in the Islamic world, especially as embodied in the institution of taqlîd, whereby a lay person blindly follows a religious scholar. We will compare this to the mystical tradition of Ibn ‘Arabî as well as the early esoteric Shî’ite tradition, where a much more “rebellious” type of Islam was offered and provided purviews of pluralism and universalism that challenge authoritarian closures of interpretation in relationship with God. In this Chapter, Maria Kronfeldner discusses whether psychological essentialism is a necessary part of dehumanization. This involves different elements of essentialism, and a narrow and a broad way of conceptualizing psychological essentialism, the first akin to natural kind thinking, the second based on entitativity. She first presents authors that have connected essentialism with dehumanization. She then introduces the error theory of psychological essentialism regarding the category of the human, and distinguishes different elements of psychological essentialism. On that basis, Kronfeldner (...) connects historical, socio-psychological, and philosophical insights in order to show that although essentialism can act as a catalyst for dehumanization, it is not necessary for it. Examples relate to dehumanization in the context of colonialism and evolutionary thinking, to the history of dehumanizing women from Aristotle to 19th-century craniology, and to contemporary self-dehumanization and ‘lesser mind’ attribution. Batteries are a fundamental storage component due to its various applications in mobility, renewable energies and consumer electronics among others. Regardless of the battery typology, one key variable from a user’s perspective is the remaining energy in the battery. It is usually presented as the percentage of remaining energy compared to the total energy that can be stored and is labeled State Of Charge (SOC). This work addresses the development of a hybrid model based on a Lithium Iron Phosphate (LiFePO4) (...) power cell, due to its broad implementation. The proposed model calculates the SOC, by means of voltage and electric current as inputs and the latter as the output. Therefore, four models based on k-Means, Agglomerative Clustering, Gaussian Mixture and Spectral Clustering techniques have been tested in order to obtain an optimal solution. (shrink)

OBJECTIVE: This paper addresses the question of the circumstances which lead clinicians to use legal coercion in the management of patients with severe anorexia nervosa, and explores similarities and differences between such formal coercion and other forms of 'strong persuasion' in patient management. METHOD: Logistic regression and other statistical analysis was undertaken on 75 first admissions for anorexia nervosa from a sample of 117 successive admissions to an eating disorder facility in New South Wales, Australia, where an eating disorder was (...) the primary diagnosis. Admissions with other primary diagnoses, such as bulimia nervosa (25 episodes), and entries with a co morbid diagnosis (e.g. depression or opiate overdose), were discarded, leaving 96 admissions by 75 individuals. RESULTS: Resort to measures of legal coercion into treatment was found to be associated with three main indicators: the patient's past history (number of previous admissions); the complexity of their condition (the number of other psychiatric co morbidities); and their current health risk (measured either by Body Mass Index or the risk of re-feeding syndrome). CONCLUSIONS: Our study is consistent with the few earlier studies about indicators for legal coercion in anorexia nervosa management, and suggests that clinicians use legal coercion very sparingly, distinguishing legal coercion from other forms of close clinical management of patients. Maria Kronfeldner’s Preface and Introduction to the Routledge Handbook of Dehumanization maps the landscape of dehumanization studies. She starts with a brief portrayal of the history of the field. The systematically minded sections that follow guide the reader through the resulting rugged landscape represented in the Handbook’s contributions. Different realizations, levels, forms, and ontological contrasts of dehumanization are distinguished, followed by remarks on the variety of targets of dehumanization. A discussion on valence and emotional aspects is added. Causes, functions, (...) and consequences of dehumanization, and the prospects for reducing or undoing it, are introduced. The systematic overview closes with a discussion of some important theoretical complexities that arise in studying dehumanization. After these systematic sections, the scholarly work on dehumanization gets situated in the broader intellectual landscape of debates about the ‘human’ in the humanities and social sciences. Changing Generations, a study of intergenerational relations in Ireland undertaken between 2011 and 2013 by the Social Policy and Ageing Research Centre, Trinity College, Dublin, and the Irish Centre for Social Gerontology, NUI Galway, used the Constructivist Grounded Theory method to interrogate support and care provision between generations. This article draws on interviews with 52 women ages 18 to 102, allowing for simultaneous analysis of older and younger women’s perspectives. The intersectionality of gender and class emerged as central to the (...) analysis. Socioeconomic positions shape contrasting forms of interdependency among family generations, ranging from “enmeshed” lives among lower socioeconomic groups to “freed” lives among higher socioeconomic groups. Women are initiating changes in how care and support flow across generations. Older women in higher socioeconomic groups are attuned to how emotional capital women expend across family generations can constrain women’s lives. In an expression of solidarity, older women are renegotiating the place of care labor in their own lives and in the lives of younger women. A new reciprocity emerges that amounts to women “undoing gender.” This process is, however, deeply classed as it is women in higher socioeconomic groups whose resources best place them to renegotiate care. Food systems transformations require coherent policies and improved understandings of the drivers and institutional dynamics that shape (un)sustainable food systems outcomes. In this paper, we introduce the Chilean National Organic Agriculture Law as a case of a policy process seeking to institutionalize a recognized pathway towards more sustainable food systems. Drawing from institutional theory we make visible multiple, and at times competing, logics (i.e., values, assumptions and practices) of different actors implicated in organic agriculture in Chile. More specifically, our findings (...) identify five main institutional transformative logics underpinning the interests and actions of organic actors. However, we find that the Law was not motivated by these logics and did not advance them. Rather, the Law was designed to support a market niche targeted to elite consumers and to reinforce agricultural exports. As a result, the Law constrains rather than enables the practice of organic agriculture and access to organic food by consumers, especially at the domestic level. We note that attention to institutional logics in the analysis of food systems, and specifically food system transformation, is relevant to more comprehensive assessments of the transformational potential of food systems policies. We conclude that there is a need to further consider and make visible the way in which different drivers (i.e., laws) are constituted through and by diverse, and often competing, institutional logics. Conceptions of evil have changed dramatically over time, and though humans continue to commit acts of cruelty against one another, today we possess a clearer, more moral way of analyzing them. In _Narrating Evil_, María Pía Lara explores what has changed in our understanding of evil, why the transformation matters, and how we can learn from this specific historical development. Drawing on Immanuel Kant's and Hannah Arendt's ideas about reflective judgment, Lara argues that narrative plays a key role in helping (...) societies acknowledge their pasts. Particular stories haunt our consciousness and lead to a kind of examination and dialogue that shape notions of morality. A powerful description of a crime can act as a filter, helping us to draw conclusions about what constitutes a moral wrong, and public debates over these narratives allow us to construct a more accurate picture of historical truth, leading to a better understanding of why such actions are possible. In building her argument, Lara considers Greek tragedies, Shakespeare's depictions of evil, Joseph Conrad's literary metaphors, and movies that portray human cruelty. Turning to such philosophers and writers as Jürgen Habermas, Walter Benjamin, Primo Levi, Giorgio Agamben, and Ariel Dorfman, Lara defines a reflexive relationship between an event, the narrative of the event, and the public reception of the narrative, and she proves that the stories of perpetrators and sufferers are always intertwined. The process of disclosure, debate, and the public fashioning of collective judgment are vital methods through which we make sense not only of new forms of cruelty but of past crimes as well. _Narrating Evil_ describes the steps of this process and why they are a crucial part of our attempt to build a different, more just world. Growing concern about economic inequality has generated a polarized narrative regarding the causes and consequences of extreme wealth. We contend that divided ideological positions obscure a more mundane reality about the typical wealthiest 1% households. Using data from the triennial survey of consumer finance, we demonstrate that there is substantial heterogeneity within the 1%. Contrary to public discourse, the typical 1% household does not have wealth reflective of popular rich lists, but derives a significant share of its wealth from ownership (...) and active management of small- to medium-sized private enterprise. We use these findings to shed new insights on business families’ relationship to economic inequality and open promising new areas of inquiry regarding the role of the family business in society. A striking feature of atrocities, as seen in genocides, civil wars or violence against certain racial and ethnic groups, is the attempt to dehumanize – to deny and strip human beings of their humanity. Yet the very nature of dehumanization remains relatively poorly understood. Having lost 73% of its purchasing power and 42% of it gross national product since the fall of the Soviet Union, Cuba faces a crisis with the modern agricultural system it had developed over the past 30 years. The response has been to put an alternative model into practice. The successes and problems associated with this model are discussed.

Leibniz's Metaphysics: Its Origins and Development. New York: Cambridge University Press, 2001. Pp. xiii + 528. Cloth, $80.00. Christia Mercer's massive study is aimed at unearthing the hidden roots of Leibniz's metaphysics by placing the German philosopher back in the intellectual context within which his thought first took shape. In so doing she stresses (...) the fundamental importance of Leibniz's early years for the development of his philosophy. In particular she convincingly argues for the key role played by Leibniz's early exposure to the thought of eclectic thinkers (including his teachers) who were trying to build a new philosophical synthesis centered around the key tenets of Christian theology and combining elements of Aristotelianism, Platonism, and (in at least some cases) the new mechanical philosophy of nature.One of Mercer's central theses is that the methodological and metaphysical commitments that Leibniz developed during his youth formed the bedrock of his mature philosophy (23). More precisely, her bold claim is that Leibniz's core metaphysics (including the complete concept account of substance) was constructed in 1668-71, re-examined and slightly developed in 1672-79, and then used for the rest of his long philosophical career (18, 470). The key inspirational forces driving the development of Leibniz's philosophy, in her view, had even earlier origins. In the mid-1660s Leibniz did not yet have a fully formulated metaphysics; but he already possessed very definite philosophical objectives which were to guide the development of his metaphysics and form the unstated assumptions underlying his mature thought (40). Mercer identifies these unstated assumptions as a "Metaphysics of Method," a "Metaphysics of Substance," and a "Metaphysics of Divinity," all of which she finds fully developed in his early thought (468-9).According to Leibniz's "Metaphysics of Method" (examined in part I), there exists a truth beneath the prominent philosophical schools which can and ought to be discovered. This conciliatory methodology was Leibniz's answer to the political, religious, and philosophical chaos around him. He was deeply convinced that his brilliantly original eclectic metaphysics would provide the foundation of philosophical, theological, and even political peace. Leibniz's "Metaphysics of Substance," Mercer argues in part II, melded an Aristotelian approach to substance with a mechanical physics and originated at the very outset of his intellectual carrier in an attempt to solve specific theological problems (e.g., those of the incarnation and resurrection). Platonism in turn offered the material for Leibniz's "Metaphysics of Divinity" (explored in part III), that is, for his conception of the relationship between God and creatures. The fourth and final part of Mercer's book takes her analysis of Leibniz's writings up to 1679 and describes the laying of the foundations of his mature metaphysics. She concludes not only that Leibniz proposed the doctrine of pre-established harmony prior to his departure for Paris in March 1672, but also, more generally, that the other tenets of Leibniz's mature thought, including his theory of truth, grew naturally out of his early metaphysics (see, in particular, 2, 15-8, 47, 52, 169-70, 250-2, 300-1, 472).Throughout the book Mercer describes her interpretation as "startling," "unfamiliar," "dramatic," and "surprising," and fears that "readers may balk" at her conclusions since "so much of Leibniz's thought has escaped us for so long" (see, for instance, 1, 9, 471). Postmodern political critiques speak of the death of ideology, the end of history, and the postsecular return of religious attitudes, yet radical conservative theorists such as Mark Lilla argue religion and politics are inextricably intertwined. Returning much-needed uncertainty to debates over the political while revitalizing the very terms in which they are defined, María Pía Lara explores the ambiguity of secularization and the theoretical potential of a structural break between politics and religion. For Lara, secularization means three things: the translation (...) of religious semantics into politics; a transformation of religious notions into political ideas; and the reoccupation of a space left void by changing political actors that gives rise to new conceptions of political interaction. Conceptual innovation redefines politics as a horizontal relationship between governments and the governed and better enables societies (and individual political actors) to articulate meaning through action--that is, through the emergence of new concepts. These actions, Lara proves, radically transform our understanding of politics and the role of political agents and are further enhanced by challenging the structural dependence of politics on religious phenomena. The purpose of this paper is twofold: first we outline a version of non-descriptivism, ‘minimal expressivism’, leaving aside certain long-standing problems associated with conventional expressivist views. Second, we examine the way in which familiar expressivist results can be accommodated within this framework, through a particular interpretation that the expressive realm lends to a theory of meaning. Expressivist theories of meaning address only a portion of the classical problems attributed to this position when they seek to explain why the expressions they (...) deal with have a given meaning. A position can nevertheless be termed ‘expressivist’ – in the minimal sense that we favor – based simply on the following key features of the meaning of these expressions: they can be used as functions of propositions, and they are not used to describe the way the world is. Standard accounts of derivational morphology assume that it is incremental: some words are formed on the basis of others, and each derivational family has a base from which all of the other words are derived. The importance of the base has been questioned by paradigmatic approaches to morphology, which posit that word systems are about multidirectional relationships between words and paradigm cells, in which no word has a privileged status. This paper seeks to test which of these two views makes (...) more accurate predictions about speakers’ cognitive representations of derivational families. We perform an acceptability judgement experiment in which speakers are asked to evaluate the acceptability of a pseudoword conditional on another pseudoword in the same derivational family. We find that speakers are aware of implicative relationships between words in the same family, and that they opportunistically exploit probabilistic relationships between surface words, regardless of whether the base form is the predictor, the target of prediction, or not at all involved in the task. We argue that the counterfactual representations of popular culture, like their religious cognates, are shaped by cognitive constraints that become visible when considered in aggregate. In particular, we argue that comic-book literature embodies core intuitions about sociality and its maintenance that are activated by the cognitive problem of living in large groups. This leads to four predictions: comic-book enforcers should be punitively prosocial, be quasi-omniscient, exhibit kin-signalling proxies and be minimally counterintuitive. We gauge these predictions against a large sample of (...) 19,877 characters that were derived from 72,611 comics using data scraping techniques. Our results corroborate the view that cognitive constraints exercise a selective effect on the transmission of popular culture. (shrink)

Arguments by Sorkin (Impossible measurements on quantum fields. In: Directions in general relativity: proceedings of the 1993 International Symposium, Maryland, vol 2, pp 293–305, 1993) and Borsten et al. (Phys Rev D 104(2), 2021. https://doi.org/10.1103/PhysRevD.104.025012 ) establish that a natural extension of quantum measurement theory from non-relativistic quantum mechanics to relativistic quantum theory leads to the unacceptable consequence that expectation values in one region depend on which unitary operation is performed in a spacelike separated region. Sorkin [ 1 ] labels (...) such scenarios ‘impossible measurements’. We explicitly present these arguments as a no-go result with the logical form of a reductio argument and investigate the consequences for measurement in quantum field theory (QFT). Sorkin-type impossible measurement scenarios clearly illustrate the moral that Microcausality is not by itself sufficient to rule out superluminal signalling in relativistic quantum theories that use Lüders’ rule. We review three different approaches to formulating an account of measurement for QFT and analyze their responses to the ‘impossible measurements’ problem. Two of the approaches are: a measurement theory based on detector models proposed in Polo-Gómez et al. (Phys Rev D, 2022. https://doi.org/10.1103/physrevd.105.065003 ) and a measurement framework for algebraic QFT proposed in Fewster and Verch (Commun Math Phys 378(2):851–889, 2020). Of particular interest for foundations of QFT is that they share common features that may hold general morals about how to represent measurement in QFT. These morals are about the role that dynamics plays in eliminating ‘impossible measurements’, the abandonment of the operational interpretation of local algebras \({\mathcal {A}}(O)\) as representing possible operations carried out in region _O_, and the interpretation of state update rules. Finally, we examine the form that the ‘impossible measurements’ problem takes in histories-based approaches and we discuss the remaining challenges. (shrink)

Eoin Carney | : Continuing on from recent discussions on the overlap between Paul Ricoeur’s philosophy and care ethics, this article will aim to clarify the status of practice in Ricoeur’s work. I will argue that even though Ricoeur’s philosophy is indeed marked by its “desire for a foundation,” as care ethicist Joan Tronto has pointed out, this aim is more of a fragile wager than a principle, and is always at risk of being overturned by practices and other (...) worldviews. I will demonstrate this point by arguing that Ricoeur’s hermeneutic approach to practice leads to the view that objective methods of knowledge and explanation are always grounded by the broader hermeneutic task of practical understanding and care for the self; in moral reasoning, Ricoeur’s analysis of the conflict between respect for the rule and respect for persons results in his prioritizing of respect for the singular other rather than the universal rule, meaning that the other can always disrupt and reorient universal or foundational modes of reasoning; and finally within healthcare relations Ricoeur aims to develop an alternative understanding of respect that places it in a dialectical relation with care. These practice-oriented readings of hermeneutics, morality, and respect aim to open up a dialogue between care ethics and philosophical approaches that have often been placed outside of care ethics. | : Poursuivant les récentes discussions concernant les recoupements entre la philosophie de Paul Ricoeur et les éthiques du care, cet article a pour objectif de clarifier le statut de la pratique dans l’oeuvre de Ricoeur. Je soutiendrai que même si la philosophie ricoeurienne est bien marquée par un « désir d’un fondement », ainsi que l’éthicienne du care Joan C. Tronto le souligne, cet objectif repose plus sur un pari que sur un véritable principe, et risque toujours d’être renversé par les pratiques ou d’autres visions du monde. Je démontrerai cela en soutenant trois arguments. 1) L’approche herméneutique de Ricoeur concernant la pratique mène à penser que les méthodes objectives de connaissance et d’explication sont toujours soutenues par une tâche herméneutique plus large de la compréhension pratique et du souci de soi. 2) En ce qui a trait au raisonnement moral, l’analyse de Ricoeur sur le conflit entre le respect de la règle et le respect de la personne accorde la priorité au respect de l’autre singulier plutôt que de la règle universelle. Ainsi, l’autre peut toujours perturber et réorienter l’universel ou les modes fondateurs du raisonnement. 3) Réfléchissant aux relations thérapeutiques, Ricoeur tend à suggérer une nouvelle compréhension du respect qui se place dans une relation dialectique avec le souci. Ces lectures – de l’herméneutique, de la moralité et du respect – orientées vers la pratique ouvrent la voie à un dialogue entre les éthiques du care et des approches philosophiques que l’on considère souvent éloignées de celles-ci.
